2024-25 Permission To Tryout Request

Information must be complete and accurate or the request will not be reviewed and completed.

Please use this form to Request a Permission to Tryout Form - to be used for girls who played on a Canadettes team this season and are looking to tryout in a different center for the upcoming season. Only requests received with this form will be processed. Please submit form for each Permission to Tryout you require. Requests will be processed as quickly as possible, but please allow 1-3 days for processing.

Incomplete information will result in delays in processing your request so make sure all required data is included. Players with any outstanding team and/or league fees will not be issued a PTS/PTT. Permission to Tryout forms are valid as of 12:01 am on April 17, 2024 until May 31, 2024, and from August 15, 2024 until a later date yet to be announced. No Player may participate in any OWHA sanctioned activity with a team other than her own team unless she has a release or properly executed permission to tryout form.

NOTE: Processing of forms will not begin until after the Easter Tournament
